District 66 CD Order Form

We have copies of our winter and spring performances for sale! They are split by school on two 2-CD sets. They include:

Set 1 – Lakeview

Wind Ensemble, Symphonic Band, Concert Band, Jazz Band, Lab Band, Choir

Set 2 – Prairieview

Cadet Band, Beginning Band (spring), Choir

The cost of each 2-CD set is $20.00. Please return this form to Mr. Tedeschi or Mr. Ohlsen by FRIDAY, May 30.

Checks can be made payable to Dist. 66 Music Boosters. CDs should be ready in 3-4 weeks. We will contact you when we receive them. They can be picked up at Lakeview.
