8TH GRADE DOWNHILL SKI PERMISSION SLIP

In keeping with our emphasis on lifetime sports, the Physical Education Department is offering the 8th grade an opportunity to experience downhill skiing. We will be going to Villa Olivia, Bartlett, IL in January. We will leave Lakeview between 8 and 9 am and return at approximately 6:00 pm.

The cost will be:

$25.50 for lift ticket (free lesson included)

$12.50 ski rental
                                    $ 38.00 Total

Lunches may be purchased at Villa Olivia or brought by the student.

This is an optional field trip. If your child does not attend, he/she will spend a regular school day at Lakeview. If he/she will attend, please fill out the bottom portion and return it to Mrs. Andreatta or Mr. Dvorak by the posted due date.
