2019-2020 New Student Registration Instructions and Forms

First Steps to Begin Registration:

  1. Visit the appropriate School Office: Elizabeth Ide for K-2, Prairieview for 3-5, and Lakeview for 6-8. (Please note that during summer break, from June 12 - August 11, registration is only available at the Prairieview School Office for all grade levels.)

  2. Bring a completed  Residency Verification Form along with proper documentation as outlined on the form.

  3. Bring an original birth certificate. The office will make a copy and return the original to parents.

  4. Upon proof of residency, you will be given a student ID number which you will use to complete the online portion of registration at:  New Student Online Registration.

Final Steps to Complete Registration -
Registration is NOT complete until you have submitted all of the following (due by August 1, 2019):

  • 2019-2020 Registration Fee Form (one per family) 
  • Payment or E-Pay receipt: Make checks payable to Center Cass School District 66. Payments can also be made online through E-Pay (for more information on E-Pay click here). 
  • Complete all necessary forms for the grade level your child is entering. See below for a list of all required forms for each grade level.

*Athletic Physical Examinations
 are required for all 6th-8th grade students wishing to participate in the interscholastic athletic program at Lakeview Junior High School. They must have an Athletic Physical Examination Form signed by a doctor on file prior to tryouts

Lunch Program and Electronic Payment System:
Center Cass offers a hot lunch on every full day of school. Monthly menus are accessible from a link on the home page of each individual school. Click here for more information on payment options.

Waiver of Registration Fees:
District 66 ensures that each student has equal access to the programs and services offered as a part of the educational experience. A student shall be eligible for a fee waiver when the student currently lives in a household with a low income and/or a unique hardship. Please use the Fee Waiver Form to request financial accommodations.

Free/Reduced Lunch:
Free and reduced-priced meals are available to children whose family qualifies based on the size of household and income level. The household eligibility guidelines, as well as the policies governing this program, are determined annually by the State of Illinois and the federal government. Prior to the beginning of the school year, the District receives notice from the Illinois State Board of Education for families who have qualified for free meals through a Direct Certification process. Other families may become eligible for meal benefits by completing an Application for Free and Reduced Meals. Families must re-apply for meal benefits every year.
