Registration 2022-2023 for New Students/Kindergarten

New Student/Kindergarten Walk-in Registration Hours:

First Steps to Begin Registration:

Please bring the following documents to the appropriate school office (see registration hours above):

  1. Residency Verification Form  along with three proofs of residency, as outlined on the form

  2. Original birth certificate - The office will make a copy and return the original to you.

  3. Registration Information Form

  4. Home Language Survey.

  5. For Kindergarteners only, bring a completed Kindergarten Student Information form.

  6. Upon proof of residency, you will be given a student ID number which you will use to complete online registration at:  New Student Online Registration.

Final Steps to Complete Registration - Due by July 28, 2022:

For your convenience, a registration drop box will be located in the entryway of Prairieview School from June through July. Or if you prefer, mail payments and paperwork to: Center Cass School District 66, 699 Plainfield Rd., Downers Grove, IL 60516.

*Athletic Physical Examinations are required for all 6th-8th grade students wishing to participate in the interscholastic athletic program at Lakeview Junior High School. They must have an Athletic Physical Exam Form signed by a doctor on file prior to tryouts.
